Community Transitions is located in Rapid City. "Community Transitions has been providing brain injury rehabilitation services in South Dakota since January of 1988, focusing on cognitive therapy, counseling, employment and independent living. Community Transitions is the oldest program of its kind in South Dakota and is one of the oldest in the country.

In April 2008, Community Transitions began providing cognitive therapy and counseling through telerehabilitation. We are exploring new ways to bridge gaps in rehabilitation and provide quality services to people with brain injury."
